Responsibilities:
Performing visits as needed and completing coordination of client care Point of contact for all disciplines involved with providing care to patients and oversees the frequency of visits for the episode.
Consults as needed with the physician and the office giving details about patient care Performing accurate OASIS collection, ensuring the medication profile remains current, ensuring lab values have been reported to the physician timely, and attending weekly case conferences and monthly case manager meetings are key responsibilities of this role.
RN Nurses must meet the following requirements:
RN licensed in Texas or in accordance with the Texas Board of Nursing rules Minimum 2 years of home health or community experience as a registered nurse.
Kinnser experience is preferred, but we can train you Wound Vac/Wound care / Infusion therapy experience is a huge plus Job Type:
Full-time/ Part-time/PRN Home health visits (not the private duty) All Nurses need to wear a mask and temperature checks are done during personal interviews.
We are also conducting phone and Zoom interviews.
Must have a minimum of 1-2 years of nursing experience Must have reliable mode of transport Previous Home Health experience preferred Job Type:
